OVER THE LAST several years, there has been a lot of dialogue concerning what “health care value” is. The Baton Rouge Clinic is not only a pioneer, but also a leader in the redefining of this term. To maximize the end result, health care value, the Baton Rouge Clinic has been improving each component of the formula:

Patient Access + Patient Satisfaction + Quality of Care + Cost = Health Care Value

Improved access has been accomplished by adding new physicians and services, as well as expanding hours. The Clinic offers same-day appointments, and the pediatric department is now open until 8 p.m. Monday through Thursday. Pediatrics continues to be open from 8 a.m. to 12 p.m. on Saturdays, and a pediatrician is available on Sundays at the Urgent Care Center which is adjacent to the Main Clinic. Through these and many other accessibility efforts, the Clinic continues to have a low hospital admission rate and a below-national rate for 30-day readmissions to hospitals. 

The Baton Rouge Clinic has continued to focus on quality outcomes and population health. The Clinic continues to expand and improve on its use of the EMR, meeting the requirements for “Stage 2 of Meaningful Use.” Only 1% of physicians in the country have achieved this status! With the addition of Healthy Planet, a quality population health module of Epic’s EMR system, the Clinic will be able to improve care for larger and larger populations. At present, the Clinic has over 250,000 patients and continues to add over 20,000 new patients a year. Even with the continual addition of new patients, satisfaction surveys and scores have continued to remain strong, with the overall satisfaction score for the Clinic at 97%, ―significantly higher than regional and national benchmarks. 

This patient-centered, value-based approach to health care has allowed the Clinic to receive numerous recognitions for its results and innovations. Focusing on HEDIS (Healthcare Effectiveness Data & Information Set) outcomes, the Clinic has reached high benchmarks for colorectal cancer screening, breast cancer screening, high-risk medication monitoring, cholesterol control and diabetes management. The Clinic was one of 30 organizations nationally selected by the CDC as a Million Hearts Hypertension Control Champion. This award recognizes those organizations that successfully achieved blood pressure control for at least 70% of its patients through innovation in health information technology, EMR, patient communication and team approach to care. The Clinic also received another national recognition for hypertension control, reaching the 76% success rate in the American Medical Group Foundation’s “Measure Up, Pressure Down” initiative. This effort put the Clinic in the top quarter of the country. The Clinic also continues to maintain the NCQA recognition for diabetic control, along with having an ADA (American Diabetes Association) accredited Diabetic Education Center.

The Baton Rouge Clinic participates in an Accountable Care Organization with Blue Cross and Blue Shield and Cigna. In both programs, the Clinic had better quality and cost outcomes than the market. The BCBS Quality Blue Program recognized the Clinic for the highest overall performance in the state of Louisiana. The Clinic also participates with Humana in its model Medicare Advantage Program, achieving similar quality and cost outcomes. Continually working to collaborate with others, The Baton Rouge Clinic recently signed a multiyear contract with a national Medicare management services organization. The Clinic has set a goal to be the most cost-effective, high-quality provider of Medicare services in the region.

The Clinic also has entered into a national collaborative with CVS Pharmacies to improve patient care. A Baton Rouge Clinic patient can go to any CVS Minute Clinic in the country, and the information from that encounter is automatically transmitted back to that Baton Rouge Clinic patient’s physician. This is possible because both CVS and the Baton Rouge Clinic are on the Epic EMR system.

Besides being an innovative, collaborative leader in transforming the health care delivery system, the Clinic continues to give back to the community through St. Vincent de Paul’s Fill a Prescription Campaign, Pat’s Coats for Kids and the Feed a Family Campaign to name a few.

Through all of these efforts, The Baton Rouge Clinic continues to improve quality and costs, thus providing greater value to its patients.
